Insurance Information

Please bring your health care insurance information including your identification and insurance card to every visit. We will keep a scanned copy of both on file.

Wellness Pediatrics participates with several insurance companies for the convenience of our patients. It is the responsibility of the assigned account holder to be aware of your insurance benefits. Benefits, deductibles, co-pays, and maximums vary among each contract and policy.

We highly recommend that you contact your insurance company directly to confirm your coverage, and to confirm our office’s status within their network. Our billing department will file claims to any insurance carrier with whom we are a participating provider.
